InjectIVF, Concierge Services
The Client and Service Provider (InjectIVF), herein referred to as the parties, enter into this In-Home Nursing Services Contract. The purpose of this contract is to set forth the terms and conditions between the parties and terms of services the Client can expect from the Service Provider.
In consideration of valuable exchange, the parties hereby agree to the following:
I. Liability: Client agrees to hold Service Provider free of any liability as a result of injections. While the Service Provider will provide services as requested to the client in accordance with their Physicians instructuction, the Service Provider does not guarantee any specific outcome as a result of their service.
II. Insurance Coverage: We do not accept insurance coverage at this time. All payments to Service Provide must be made by the client to the Service Provider. The Client may request an invoice to provide to their insurance company if seeking reimbursement.
III. Licensing: The Service Provider warrants that all employees providing injection services as described below are properly licensed with the state of California and are current on all their required training and certifications.
IV. Attorney Fees and Litigation: The parties agree that any litigation as a result of a breach of this contract will be handled through arbitration. The party responsible for the breach of this contract is responsible for all costs incurred.
Terms of Service:
I. Description of Services: The parties agree that the Service Provider is to administer prescribed IVF services in accordance with the Client's Physician's instructions.
II. Payment: Payment of services is due as described in the invoice provided to the client at the time of scheduling their service. With the client's consent, payment information may be stored for future purposes or ongoing treatment. Failure to make payment for services rendered may result in the termination of the parties relationship.
III. Client Hindrances: To protect both parties, the Client is expected to in no way hinder or interfere in the Service Providers administration of prescribed medical treatment or the activities surrounding it. This protects both parties from clinical errors that may arise and ensures a safe, positive experience for both parties.
IV. Cancellation Policy: With the exception of pregnancy and the directive of the client's physician, a minimum 24 hour notice is required for any cancellations for scheduled appointments. Failure to cancel within 24 hours may result in a charge of 50% of the scheduled service via the payment information on file.
V. Scheduling: While the Service Provider will always do their best to meet the client's scheduling needs, the parties understand that appointments will be scheduled based on availability.
Privacy Policy:
InjectIVF takes your privacy seriously. All information collected and obtained about the Client, their affiliates, family and relatives during the parties relationship, including but not limited to legal, financial, medical, career and assets are strictly confidential and may not be disclosed to any third party for any reason. The obligations of your Service Provider under this clause survive termination of this contract.
We use Formspree to collect and process any data submitted via our contact forms. This may include your name, email address, and any information you provide in your message. Formspree may store this data securely and has its own privacy practices, which you can review in their Privacy Policy. InjectIVF will only use this data for communication and will not share it with other third parties.
